Im stuck on step 5 on UKC litter form.?

if i don't fill this step out they wont send me the birth certificates for the puppies? Or do i fill it out and don't put the puppies name because i want the buyer to name the puppy the name they want. im just really confused because when i bought my puppy the person gave me the birth certificate and i got to name my dog. i want to do the same with the buyers of my puppies. so can someone help me out please or break it down to me how to fill out the form. thank you
